If you are new to our practice, we ask that you arrive 20 minutes early to fill out our patient history forms. If you are transferring from another practice, please make arrangements to have a copy of your records and x-rays sent to us. We can help guide you through this process. Just call our office, and we’ll be happy to help.
If you are unable to keep your appointment for any reason, please notify our office a minimum of 48 hours in advance. We’ll reschedule at a more convenient time.
